Please complete each of the below forms that are required to attend camp. A parent or Legal Guardian must complete the forms. All campers must have a completed set of forms on file to remain at camp.
You will also find helpful information to assist in packing and getting to camp!
Reminder dates and times for Camp Echo:
- Dates: Monday July 27, 2025 – Friday August 01, 2025
- Arrival time: Monday July 27th 2:00 – 4:00.
- Departure Time: Friday August 1st 11:30 – 12:00
- Note: Please do not plan to arrive earlier than 2:00pm. Our Medical staff will not be prepared to receive campers until 2:00pm. You will have to wait at the Welcome Center until that time.
- Only one adult will be allowed to go to the Med Shed with the camper.
- Please be prompt on Friday for pickup Friday 11:30 – 12:00.
The following forms are required before the camper can remain at camp. These forms can be completed electronically by clicking on the name for each form.
1. Medical History forms – It is very important that you give complete information. This information is needed for our medical staff to provide care.
2. Medications Form – This will help the medical staff monitor and dispense the medications brought to camp.
- All Medication must be in the original containers. No Pill boxes.
3. Primary Care Physician and Pediatric Cardiology Consent Form –
This form must be completed by the Primary Care Physician and Pediatric Cardiology giving their consent for the camper to attend camp.

- Once completed by the primary care physician, email the form to Dawn Bressler at campechopa@gmail.com.
- Download the Cardiology Consent Form here to take to your cardiologist.
- Once completed by the cardiologist, email the form to Dawn Bressler at campechopa@gmail.com.

5. Consent / News Release Form – This form is for all releases needed for
the camper to participate in the different activities at camp.
Items to help prepare for camp:
Packing List – This list is to help you pack the needed items for camp. Please make sure your child has the appropriate shoes and clothes for camp. Please make sure you pack everything that is needed as it is hard to provide missing items such as sleeping bags, pillows, swimsuits, jackets, jeans, etc. after the campers has arrived.
Directions to Camp Victory – Map to travel to camp. I would also suggest using the Internet such as Map Quest or Map Blaster to help with your route to camp.
Note: If you wish to write or call your son or daughter while at camp, here is the following information that is needed. If you are planning on writing a note, it is suggested you send the letter out a few days prior to camp so the letter will arrive while they are at camp. Campers love to receive mail!

Where do you go first when arriving at camp?
- Your first stop will be to register at the Welcome Center. You will drop off your luggage there.
